Question:	My school has combination locks and we have lost the chart booklet that lists all of the serial numbers and their respective combinations. How can we obtain a new chart booklet?
Answer:	To obtain a new combination chart booklet, fax your request on official letterhead along with the completed Chart Request Form to 1-800-308-9245. The cost for hardcopy charts is $50 ($100 for full end user history). A remittance request for the fee will accompany your chart. Please allow 2-4 weeks from the receipt of your request to process your chart.
